This document summarizes our business approach to fulfilling the need to complete many of our jobs. While it touches on some of the possible opportunities that have arisen for our company, these opportunities can also be expanded to those companies willing to enter into a service alliance with OSBT. All of the necessary contacts, as well as some of the requirements and procedures vital to meet this agreement, are included in this document. |

| W-9 |
The W-9 is a document we use for staffing and contracting purposes. The form delegates pay to be distributed without taxes being taken out by the government. The recipient of a payment under a W-9 agreement is personally responsible for paying the appropriate taxes out of the check to the IRS. |

OSBT uses this document to qualify our potential service partners before signing them on officially. This form makes it easy to highlight your company’s contacts, rates, capabilities, equipment, services, and qualifications. |
| Sample Insurance Form |
This is an example of a document required by OSBT to form a service alliance with us. We will refuse to partner with any company that chooses to work without proof of insurance or that withholds such forms from OSBT. Proof of your company’s insurance must be provided before an agreement can be reached. |
